Before we begin lessons, I ask each family to complete a few simple forms. These help me get to know your child, support their learning style, and ensure we’re all on the same page.

Can’t make a scheduled lesson? Submit a request here. Makeups are not guaranteed, but I’ll do my best.Withdrawal or Pause Form
Stepping away from lessons for now? This form helps close things out respectfully and keeps the door open for a return.
Privacy Reminder:
All submitted information is confidential and used solely for planning lessons, supporting your child’s progress, and maintaining communication. You’re always welcome to update your responses if your situation or preferences change.
